•One-third of people receiving protease inhibitors (PIs) develop major PI resistance.•Early HIV genotyping is required in patients failing PI treatment.•Extensive PI resistance compromises darunavir activity in third-line regimens.•When monitoring third-line treatment, early evaluation of resistance is warranted.
