Abstract
Case conceptualization is the process of developing a comprehensive understanding of a client's problem, including its causes, maintaining factors, and potential treatment goals. This understanding is based on a variety of information, such as the client's psychological, relational, and behavioral problems. Case conceptualization can be used to develop a treatment plan and strategy that is tailored to the individual client's needs. To develop the ability to conceptualize cases, counselors need to establish a primary counseling theory. Counseling theory provides concrete guidelines to prevent counselors from losing sight of the direction of counseling and to reduce confusion in the counseling process. Counseling theory is also a criterion for evaluating case conceptualization. It is therefore important for counselors to develop their own primary theoretical perspective and to secure professional competence. Despite the importance of case conceptualization in counseling, research on this topic is relatively scarce, especially research on counseling theories. There is a need for research on case conceptualization using transactional analysis theory to address these limitations. In response to this need, this study aims to clarify the content and elements of transactional analysis counseling case conceptualization. It is also necessary to confirm the difference between this study and previous studies. This study was an attempt to develop and validate the elements of transactional analysis counseling case conceptualization. The research questions were as follows First, What are the elements of transactional analysis counseling case conceptualization? second, Are the elements of transactional analysis counseling case conceptualization valid? To solve these research problems, prior research and literature on case conceptualization-related factors were reviewed, and 175 components of transaction analysis theory and 23 major concepts were extracted. To construct the extracted elements of the case conceptualization, we formed four transaction analysis consultation experts and went through a pre-sorting and a main sorting process. We selected 23 transaction analysis consultation experts and conducted three Delphi surveys on the preliminary elements. As a result, we developed 16 elements of case conceptualization for transactional analysis counseling, which consists of 7 categories. The specific elements include, ‘Problem characteristics’ presenting problem, symptoms, triggers, background information, ‘Resources’ strengths and positivity, ‘Contract’, therapeutic and behavioral contract, ‘Script formation’, origin of the script, early experiences, early decisions, ‘Script maintenance and reinforcement’ personal factors, relational factors, situational factors. The details of the personal factors include ego state structural analysis, drivers, discounts, and redefinition. The details of the relational factors include ego state functional analysis, communication pattern analysis, strokes, time structure, game analysis, and symbiosis. The details of the situational factors include personality adaptation and framework. The "comprehensive understanding" category includes comprehensive understanding and analysis, racket system analysis, and script analysis. The "counseling goals and plans" category includes agreed-upon goals, clinical goals, and counseling goals and plans. The details of counseling strategies and interventions include the following, Allow a script prohibition order Drive out the CP ego, Ego state insight, Find discounts, Activate the adult ego, Challenge the frame of reference, Empty chair technique, Reciprocal conversation, Remove ego state contamination, Reparenting, Stroke exchange, Close the escape route, Redecision, Option, Stop the game, Reset the time structure. This study developed and validated the elements of case conceptualization for transactional analysis counseling. This study is meaningful in that it developed the elements of case conceptualization for transactional analysis counseling based on the consensus of transactional analysis counseling experts in a reality where there is no research on case conceptualization for transactional analysis counseling. In addition, this study analyzed a hypothetical case that mixed actual cases and presented an example of a case conceptualization method. The results of this study are expected to be helpful for the understanding and application of case conceptualization for transactional analysis counseling. The elements of transactional analysis counseling case conceptualization are expected to be clinically useful in terms of how to structure case conceptualization by integrating the practice and theory of counseling. This process can be a good example of therapeutic intervention to understand and analyze the client. This is expected to improve the level of transactional analysis counseling case conceptualization and promote the professional growth of transactional analysis counselors.
