幼年期与成人期起病的强直性脊柱炎临床特点比较 / Comparison of clinical characteristics between patients with juvenile and adult ankylosing spondylitis

Abstract
目的:分析幼年期起病的强直性脊柱炎(JAS )与成人期起病的强直性脊柱炎(AAS)在起病方式、临床表现、影像学表现上的异同。方法收集576例强直性脊柱炎(AS)患者(其中 JAS 165例,AAS 411例)的临床资料,比较2种 AS 患者在起病方式、临床表现、影像学表现上的异同。结果与 AAS 相比,JAS 多以外周关节症状起病,而 AAS 多以腰骶部症状起病;JAS 髋关节受累的比率(69.1%)明显高于 AAS (30.9%),比较差异有统计学意义(P <0.01);JAS 脊柱受累比率(37.0%)明显低于 AAS (64.2%),比较差异有统计学意义(P <0.01)。JAS 患者眼炎的发生率(4.9%)明显低于 AAS (11.4%),比较差异有统计学意义(P <0.05)。骨密度检查示 JAS 髋骨骨密度明显低于 AAS,比较差异有统计学意义(P <0.05)。结论AS 多以外周关节起病,髋关节受累常见,而 AAS 多以腰骶部症状起病,脊柱受累多见;JAS 髋骨骨密度减低较 AAS 重,而并发眼炎的发生率低于 AAS,临床医生需关注 JAS 和 AAS 的不同,以便早期诊断、积极处理和预防并发症。
Objective To compare the type of onset,clinical presentation and imaging manifestations between Chinese Han patients with juvenile ankylosing spondylitis (JAS)and adult ankylosing spondylitis (AAS)from southern China.Methods Clinical data of 576 patients diagnosed with ankylosing spondylitis (AS),including 165 JAS and 411 AAS,were obtained.The type of onset,clinical presentation and imaging manifestations were statistically compared between JAS and AAS patients.Results Compared with AAS pa-tients,most JAS individuals presented with peripheral arthritis as type of onset,whereas most AAS counterparts had lumbosacral pain as initial symptoms.JAS patients had a significantly higher rate of hip joint involvement (69.1%)than AAS counterparts (30.9%)(P <0.01).The incidence of spinal involvement in the JAS pa-tients (37.0%)was significantly lower compared with 64.2% in the AAS counterparts (P <0.01).The inci-dence of ophthalmia in the AAS group (4.9%)was significantly lower than 11.4% in the AAS group (P <0.05).Bone mineral density (BMD)test revealed that the BMD at the hip in the JAS patients was significant-ly lower compared with that in the AAS counterparts (P <0.05).Conclusions JAS patients were mainly characterized with peripheral arthritis as type of onset and hip joint involvement.AAS patients mainly presented with lumbosacral pain as initial symptoms and spinal involvement.The BMD of hip in the JAS patients was more significantly reduced compared with that in the AAS counterparts,whereas the incidence of ophthalmia in the JAS group was considerably lower than that in the AAS group.Clinicians should emphasize upon different manifestations between JAS and AAS patients to make early diagnosis,take active measures and prevent the in-cidence of complications.
