Abstract
This document is a reply to a comment made by Belletti et al. regarding an article titled "Standardized approach for extubation during extracorporeal membrane oxygenation in severe acute respiratory distress syndrome: a prospective observational study." The authors of the reply commend Belletti et al. for their systematic review on the outcomes of awake extracorporeal membrane oxygenation (ECMO) without invasive mechanical ventilation (IMV) in patients with respiratory failure. They agree that active physiotherapy may provide additional benefits when weaning patients from IMV in ECMO. The authors also discuss baseline parameters on multi-organ failure, the evaluation of right ventricular function, and the potential role of early detection of the Macklin effect in tailoring respiratory support for patients with acute respiratory distress syndrome (ARDS). They suggest that future studies should further investigate these factors to refine the selection and stratification process for patients who could benefit from ECMO without IMV.
