Abstract
Since confirmed VZV vasculopathy does not always cause a rash, future work should identify whether CSF analysis for VZV antibodies and IgG index is useful in the investigation of unexplained ("cryptogenic") ICH. The timing of VZV reactivation and subsequent ICH occurring at a peak risk interval, as well as the lack of any alternative explanation for ICH after detailed investigation, points to a probable causal association between VZV and ICH. In summary, to the best of our knowledge, this is the first report to describe likely VZV small vessel vasculopathy associated with isolated deep ICH, with detailed negative cerebral vessel imaging and presence of VZV IgG in the CSF.
