Health and Social Care coordination in Andalusia within the European project VIGOUR.

Abstract
Introduction: Andalusia, a Spanish region of 8.4 M inhabitants is the location of one of the pilots in the European project VIGOUR, which aims at scaling-up integrated care in several regions in Europe. In this pilot, better coordination between health and social care services in the region will be achieved through improved digital communication. The entire population is covered by the regional public healthcare system (the Andalusian Health Service) with a massive use of eHR at all levels of care. Social services are provided by a network of community and specialised services (local and regional level). Aims and Objectives: Based on previous collaboration, a stronger coordination between health and social services will be achieved thanks to improved information and communication channels. This coordination is focused on the process of recognition of the dependency situation, managed by social services and requiring a health report issued by health services. Both digital platforms are in the process of improving their interactions to request and access form the social services one to the health report within the health services solution. Improved satisfaction by health professionals with the new health report will be assessed. Reduced response times are expected as well. Key Findings: A new health report is issued within the corporate electronic health record (Diraya) in the Andalusian Health Service (SAS). The health report is requested and retrieved by the Dependency Management System of the Andalusian Agency for Social Services and Dependency (ASSDA). Healthcare professionals, mainly nurses, at primary care level will be using the new feature, initially in two provinces in the region (Huelva and Jaen). The level of acceptance of this new feature will be assessed using a standardised questionnaire, allowing for additional feed-back from professionals to improve de final digital solution. Reduced delays in the process of the recognition of the dependency situation are expected too. Conclusions: Better communication between health and social services is possible thanks to digital solutions based on the current corporate systems in the region of Andalusia. A coordination committee at political level also supports the process to achieve improved results and reduced response times in the process of dependency situation assessment. Implications for sustainability: Improved coordination between social and health services in the region of Andalusia will be extended to the entire region, following the initial experience in two provinces.
