Abstract
The article discusses how to effectively transition teenagers and young adults with intellectual or other developmental disability (IDD) into adult healthcare. Other topics include the three phases of transition, namely, preparation and integration into adult-based care, the importance of systematic health checks like lifestyle counseling, and the Got Transition system created by the National Alliance to Advance Adolescent Health.
