Abstract
For anatomical and biomechanical reasons, thoracolumbar fractures are very common injuries. Although the surgical management of these fractures is still an open issue, the main goals of the treatment are always represented by restoration of vertebral stability, correction of kyphotic deformity, decompression and protection of neurological structures and a fast functional improvement.The surgical treatment of this kind of fractures needs a precise understanding of the traumatic mechanism. Posterior instrumentation, that includes different surgical techniques, allows the treatment of the majority of vertebral lesions and indirectly involves also the anterior region of the spine, avoiding the possible complications of an immediate anterior approach without excluding the possibility of a subsequent anterior somatic reconstruction.
