The Maastricht UMC+ Academy for Patient and Informal Caregiver was founded to optimally support patients and informal carers’ health related self-management. A palette of activities offers patients and informal carers the opportunity to increasingly participate as fully-fledged partners in their care team. In addition, the Academy for Patient and Informal Caregiver aims to create value based on the so-called quadruple aim: increased health as a result of independently carrying out self-management tasks in an appropriate and safe manner that prevents referrals to professional home care provision, shortens hospital stays and ensures sustainable deployment of healthcare professionals.
