Abstract
The AGA-Society for Arthroscopy and Joint Surgery treatment algorithm for patellar instability (ATAPI) is based on the clinical examination, imaging and the resulting risk profile. In the first step, a possible flake fracture is detected by imaging. In the second step the individual risk for patellar re-dislocation is quantified depending on the anatomical and epidemiological parameters present. Conservative treatment is primarily recommended in cases of a low-risk profile with no evident flake fracture. If a flake fracture is detected, refixation of the fragment is strived for. An additional reconstruction of the medial patellofemoral ligament (MPFL) significantly reduces the risk of re-dislocation and represents the main pillar of the operative treatment of patellar instability. Other additional interventions should be taken into consideration depending on the individual risk profile. Therefore, operative treatment is also indicated even in cases of patella dislocation with a high-risk profile but no flake fracture.
