Laparoscopic Roux-en-Y Gastric Bypass Versus Laparoscopic Sleeve Gastrectomy: a 5-Year Follow-Up Prospective Study of 200 Cases

Abstract
To compare outcomes of laparoscopic Roux-en-Y gastric bypass (LRYGB) versus laparoscopic sleeve gastrectomy (LSG) in patients with body mass index (BMI) ≥ 35 kg/m2 in terms of percent of excess weight lost (%EWL), percent of excess of body mass index lost (%EBMIL), postoperative complications, and resolution of comorbidities. A prospective and comparative study was carried out. Patients who met the inclusion criteria (200) were analyzed and assigned to two groups (LRYGB = 100 and LSG = 100), underwent surgery between 2012 and 2013, and followed up to 5 years. The mean follow-up was 66.5% (133 patients). Both groups were initially comparable in number of patients and age, not so in BMI and excess weight (p = 0.001). %EWL in LRYGB group was 76.3 ± 15.3, while in LSG group was 71.7 ± 21.9 with no significant difference (p = 0.172). Postoperative BMI was 27 ± 3.5 and 26.5 ± 5.1 kg/m2 for LRYGB and LSG, respectively, without significant difference (p = 0.527). Upper gastrointestinal bleeding was the most frequent complication in both groups, with no significant difference. Regarding resolution of comorbidities, hypertension remission was statistically significant in favor of LRYGB. No differences were found in the rest of comorbidities studied. The study showed successful %EWL, %EBMIL, and resolution of comorbidities, with very low morbidity and no mortality for both techniques. Only hypertension had significant better outcomes within LRYGB group. No clear advantage in any of these two procedures could be determined after 5 years of follow-up.
