(Anaesthesia. 2018;73:480–489)Long-acting neuraxial opioids provide excellent analgesia following surgery, but they have also been known to contribute to postoperative nausea and vomiting, thereby negatively affecting postoperative recovery and patient experience. Intravenous (iv) dexamethasone is an effective antiemetic against postoperative nausea and vomiting associated with general anesthesia, but its efficacy in preventing nausea and vomiting related to the administration of long-acting neuraxial opioids for postoperative analgesia remains undetermined. The aim of this systematic review and meta-analysis was to assess the efficacy of iv dexamethasone as a prophylactic antiemetic in surgical patients receiving long-acting intrathecal or epidural opioids.
