Succesfull treatment of supralethal caffeine overdose with a combination of lipid infusion and dialysis☆

Abstract
We describe a life-threatening caffeine overdose (peak plasma level 310 mg/L) caused by caffeine tablets obtained over the counter. Patient presented with a triad of cAMP-related symptoms: refractory ventricular tachycardia, hypotension, and severe distributive hypokalemia. The management consisted of a combination of intravenous lipid infusion and continuous hemodiafiltration, uniquely targeting both hydrophilic and lipophilic moiety of the molecule. This resulted to a sharp decline of caffeine plasma levels as well as resolution of clinical symptoms.
