Abstract
Transplantation presents several unique communication roles for the palliative care provider, including patient and family assistance with healthcare decision-making about transplantation itself. The communication challenges regarding transplantation in conjunction with palliative care, however, extend well beyond that. They include assisting the patient and family with legal issues such as advance directives, communicating the prognosis, making decisions about life-sustaining treatment in combination with or without palliative care, potential reliance on hospice, and the role of life-support interventions. Many of these challenges are issues with which transplantation specialists may have little experience or expertise, just as the transplantation issues may be relatively new to palliative care professionals.
