Abstract
Around one-quarter of bladder cancers are muscle invasive (MIBC). These cancers have a poor prognosis; hence, prompt and accurate diagnoses are needed to maximize curative outcomes. MIBCs are best managed within multidisciplinary teams. Many patients have pre-existing comorbidities and need counselling regarding systemic chemotherapy, radical surgery (with reconstructive choices), or radiotherapy. Despite treatment, metastatic disease develops in up to half of all patients and so palliative support is also needed. Through the presentation of this difficult case—MIBC in a gentleman with a solitary kidney—some of the challenges faced in managing MIBC are explored.
