Depressive symptom, sleep duration and quality of life in Korean menopausal women: The Korea National Health and Nutrition Examination Surveys (KNHANES VI)

Abstract
Objective: Women are more vulnerable than men to depressive disorder. Hormonal effect contributes to the gender difference in the prevalence of depressive disorder. Particularly, middle-aged women with abrupt hormonal fluctuation are susceptible to depression, and complain about sleep disorder and low quality of life as well. Until now, as a few studies about mental health of perimenopause and postmenopausal women was reported in Korea, we evaluated the relationship between the prevalence of depressive symptoms, short sleep duration and low quality of life and the period of menopause. Methods: The study included a total of 2503 premenopausal and postmenopausal women who had completed information on Patient Health Questionnaire 9 (PHQ-9), sleep duration and the EuroQol five-dimensional (EQ-5D) index in the 6th Korean National Health and Nutrition Examination Survey (KNHANES VI). We divided the women with four groups according to the period of reproductive age; premenopause women, early menopausal women (under 5 years after menopause), late menopausal women (over 5 years after menopause) and geripause (over 65 years old). We adjusted rst for age (model 1), and then for age, BMI, income, education, smoking status, alcohol use, exercise (model 2). Results: After adjustment for age(model 1), geripause women had a significantly higher score of PHQ-9 than other groups(4.74±0.42 vs 2.46±0.26, 3.13±0.32, 3.21±0.22; p=0.004). Compared with premenopausal women, the risk of depression(5 of PHQ-9 score) was significantly as the period after menopause increased(OR 4.42, 95% CI=2.20-8.86; P<0.001). In model 2, the risk of depression was also significantly higher as the period after menopause increased (OR 3.35, 95% CI 1.67-6.72; p=0.004). Sleep duration and EQ-5D were not significantly associated with menopausal period. Conclusion: Compared to premenopausal women, the risk of depression increases as the period of menopause increases in Korean women, particularly highest in geripause women over 65 years old.
