Adherence to diabetes management recommendations of a Pay-For-Performance Program was associated with blood pressure control for type 2 diabetic patients in Taiwan

Abstract
Objective: In Taiwan, a Diabetes Pay-For-Performance Program was implemented by National Health Insurance since 2001. This Diabetes Pay-For-Performance Program included multi-displine professionals as an integrated care team and provided continuous care with management recommendations and regular follow-up. The objective of this study was to investigate whether adherence to diabetes management recommendations of the Pay-For-Performance Program was associated with blood pressure control in type 2 diabetic patients. Methods: A total of 521 patients who join the Diabetes Pay-For-Performance Program were enrolled in this study in one tertiary teaching hospital (1,300 beds) in Taichung, Taiwan, 2002-2012. Demographic characteristics, DM duration and anthropometric measures, including body mass index, systolic blood pressure (SBP), diastolic blood pressure (DBP), and body weight, were collected for all patients. We stratified the adherence rate to diabetes management recommendations of the Pay-For-Performance Program into >= 80% and < 80% groups. In addition, we stratified the follow-up period into between 1~2 years and between 2~3 years. Wilcoxon rank sum test was used for statistical analysis. Results: In the follow-up period between 1~2 years group, adherence rate to diabetes management recommendations ( >= 80% v.s. < 80%) was associated SBP reduction (-3.0 mm Hg v.s. -1.9 mm Hg, p = 0.03). DBP showed no difference after the Pay-For-Performance Program. Conclusion: Higher adherence to diabetes management recommendations of a Pay-For-Performance Program was associated with better blood pressure control for type 2 diabetic patients in Taiwan.
