Introduction: Systemic inflammatory response syndrome (SIRS) is a serious condition related to systemic inflammation, organ dysfunction, and organ failure. It is a subset of cytokine storm, in which there is abnormal regulation of various cytokines. SIRS is also closely related to sepsis, in which patients satisfy criteria for SIRS and have a suspected or proven infection. The adult SIRS Criteria were following: Body temperature less than 36°C (96.8°F) or greater than 38°C (100.4°F), Heart rate greater than 90 beats per minute. Tachypnea (high respiratory rate), with greater than 20 breaths per minute; or, an arterial partial pressure of carbon dioxide less than 4.3 kPa (32mmHg) and White blood cell count less than 4000 cells/mm³ (4 x 109 cells/L) or greater than 12,000 cells/mm³ (12 x 109 cells/L); or the presence of greater than 10% immature neutrophils (band forms). Band forms greater than 3% is called bandemia or a "left-shift."In this study, we systemic analysis the percentage of immature cell in SIRS and try to set up a cut off level to diagnosis. Materials and Methods: In this study, we collected 1493 cases and analyzed the percentage of immature cell. By using the microscope, we check the immature cell of each 100 white blood cell. Results: After calculated, the average of WBC was 17435 ± 4396 x 109 cells/L. And the percentage of immature cell was 8.5 ± 3.7 %. Most of the immature cell belong to band form, some of them were metamyelocyte and mylocyte. But in theory, once metamyelocyte presented, the stage of infection must be bacteremia or sepsis. So the data seem a little strange that we have to educate Conclusion: Depend on this study, we suggested every hospital can conduct a lecture to educate the staff. And try to set up a cut-off data to alarm that could be SIRS. The cut-off day should not obey by the adult SIRS Criteria, it must create by own lab.
