Abstract
ObjectivesThe aim of the study was to investigate the efficacy of Bangpungtongsung-san(BT) and Bangkihwangki-tang(BH) for obesity. We examined the efficacy of BT and BH would vary according to the single nucleotide polymorphism(SNPs). We aimed that oriental obesity pattern identification could be a diagnosis tool to predict efficacy and adverse reactions of herb medicine. Methods166 subjects (body mass index≥25m/kg2) were recruited and randomized to receive BT(n=55), BH(n=55) or Placebo(n=56) for 8weeks. Anthropometric factors, serum lipid profile, glucose, blood pressure(BP), pulse rate, resting metabolic rate, oriental obesity pattern identification questionnaire and Korean version of obesity-related quality of life(KOQOL) scale measured at baseline and 8weeks. SNPs (β3-adrenergic receptor(ADRB3), G protein β3(GNB3), peroxisome proliferator activated receptor gamma 2 gene(PPAR-γ2), uncoupling protein(UCP2)) were conducted at baseline.Adverse reactions and safety outcome variables were also checked during trials. ResultsAll three groups showed significant improvement on obesity after treatment. BT group decreased total cholesterol and BH group decreased triglyceride whereas increased HDL cholesterol than did BT group and Herb medicine improved KOQOL.According to obesity pattern identification, BT group significantly decreased body weight, body mass index(BMI), waist circumference(WC), body fat mass, total cholesterol and HDL-cholesterol in Stagnation of the liver Qi(肝鬱) and waist circumference(WC) in Indigestion(食積). BH group showed significant decrease of body weight, BMI, WC, and body fat mass in Indigestion(食積) not in Deficiency(虛症). Adverse reactions were reported most frequently in Yang deficiency(陽虛) by BTS group and Stagnation of the liver Qi(肝鬱) by BH group. BT and BH showed a significant higher efficacy in C/T and T/T genotype of GNB3 gene / in Trp64 and Arg64 genotype of ADRB3 gene / in D/D genotype of UCP2 gene / in Pro/Pro genotype of PPAR-γ gene. ConclusionBT and BH promoted obesity indexes and obesity pattern identification could be a useful diagnostic tool predicting treatment effects and adverse events. Pharmacogenetical studies of BT and BH on obesity could be a effective method for the individualized treatment and prevention of obesity.
