Abstract
Background & Objective: Several studies have been conducted on olfactory changes after endoscopic sinus surgery. Because the difference of the olfactory tests and postoperative follow-up period, there were mixing results on postoperative olfactory changes. Therefore, we investigated postoperative olfactory changes and prognostic factors in patients with long term follow-up (≥6months) and endoscopical good wound healing, using objective and subjective olfactory tests. Methods: Thirty patients that underwent functional endoscopic sinus surgery for chronic rhinosinusitis with nasal polyposis were examined preoperatively, and at six months after operation. Osteomeatal unit computed tomography (CT) and olfactory cleft score (OCS) were obtained preoperatively and olfactory function tests such as the butanol threshold test, the cross-cultural smell identification test (CCSIT), and questionnaires were performed at the initial preoperative visit and at six months postoperatively. Results: The results of Olfactory tests, such as butanol threshold test, CCSIT, and questionnaire improved after surgery. The results of subjective olfactoryt tests, such as questionnaire I and II, were improved in 77-90%, which was higher rate than objective olfactory tests, showing 68-83%. The olfactory cleft opacification showed a correlation with the preoperative results of olfactory function tests than the CT score. Preoperative Lund-Mackay CT score and improvement of CCSIT levels showed significant negative correlation. More significant improvement of olfactory function test and questionnaires in the low groups of olfactory cleft score showed than the high score groups of olfactory cleft score. Conclusion : Postoperative olfactory improvement rate after endoscopic sinus surgery in cured patients is 63%. Among the improvement rate of objective olfactory threshold test(Buthanol threshold test) is higher than the improvement rate of perception of olfactory test(CCSIT). The postoperative olfactory tests and the improvement of olfactory scores after surgery were increased more in the mild OCS group than in the moderate and severe OCS group.
