Abstract
Liver transplantation is the most effective treatment for end-stage liver diseases (ESLD) with satisfactory clinical results and so is considered as the treatment of choice for ESLD and early hepatocellular carcinoma with cirrhotic liver. Unfortunately, adverse effects of life-long immunosuppression prohibit the development of alternative strategies to achieve better long-term outcome. Achieving clinical operational tolerance is one of the ultimate goals in the clinical transplantation field. From 1998 to 2020, 2300 liver transplantation recipients were reviewed in a single center. Nighty-nine recipients achieved operational tolerance without any rejection episodes. There was no difference between the two groups in gender and BMI of the tolerance group (TG, n=99) and non-tolerant group (NTG, n=2201). However, there was a difference in age, which was arose from the ratio of adults and children. In the proportion of children, the TG was 24% and the NTG was 10.1%, showing a remarkable difference. The proportion of living donor liver transplantation was high in the NTG. In a comparative analysis of 2054 adult patients aged 18 years or older who survived at least 1 year after transplantation during the relevant period, there was no difference in age, gender, BMI, etc. between the adult TG (n=75) and the adult NTG (n=1979). However, the rate of living donor liver transplantation was 40.3% (29/75) in TG and 84.8% in NTG (p
