코로나바이러스감염증-19 확산에 따른 응급실 기반 선별진료소 이용 및 응급실 격리병상 배정 현황 / Status of use of emergency department-based screening clinic and the allocation of emergency department isolation beds in the spread of coronavirus disease (COVID-19)

Abstract
목적: 응급실 기반 선별진료소는 코로나바이러스감염증-19 확산 이후 응급의료기관에 많이 설치되었다. 응급실 기반 선별진료소는 응급실 입구에 설치되어 감염 의심환자를 사전 차단하는 선별의 역할과 응급환자 중증도 분류, 간단한 응급처치까지 시행하는 역할을 동시에 겸하고 있는 새로운 개념의 다기능 진료소이다. 이러한 개념의 응급실 기반 선별진료소는 국내외에 정의된 바가 없다. 이에 응급실 기반 선별진료소의 이용과 응급실 격리병상 배정 현황을 분석하여 응급실 기반 선별진료소 운영에 도움 되려고 한다. 방법: 본 연구는 2020년 2월 7일부터 4월 30일까지 일개 대학병원의 응급실 기반 선별진료소를 이용한 환자의 전자 의무기록을 바탕으로 후향적으로 자료를 수집하고 분석하였다. 응급실 기반 선별진료소를 이용한 환자 중 코로나바이러스감염증-19 검사를 시행한 환자와 응급실 격리병상에 배정된 환자를 분석하였다.결과: 본 연구에 포함된 응급실 기반 선별진료소를 이용한 환자는 총 1409명이었다. 이중 코로나바이러스감염증-19 검사를 시행한 환자는 1378명이었고, 코로나바이러스감염증-19 검사 후 귀가한 환자는 1226명, 응급실 격리병상에 배정된 환자는 152명이었다. 통계 분석상 응급실 격리병상 배정에 영향을 준 요인은 고령, 14일 이내 해외 여행력, 발열 등이 확인되었으며, 응급실 격리병상은 적절하게 배정되었다. 결론: 코로나바이러스감염증-19 의심환자가 응급실 기반 선별진료소 방문 시 고령, 14일 이내 해외 여행력, 발열 등이 있는 경우 응급실 격리병상 배정을 고려해야 한다.
Introduction: Emergency department based screening clinic was regularly installed in the emergency department of medical institutions following the spread of COVID-19. The emergency department based screening clinic is a new concept; multi-functional clinic at the entrance of the emergency department. It serves as a screening center for identifying patients likely to be having COVID-19, as well as triaging patients in critical condition, and even providing basic emergency treatment. The emergency department based screening clinic with the aforementioned concept has not been defined in Korea or globally. Therefore, in this study, we aims to analyze the use of the emergency department based screening clinic and the status of allocation of the emergency department isolation beds, to help the operation of the emergency department based screening clinic.Methods: Data of patients who visited the emergency department based screening clinic of a university hospital between February 7, 2020 and April 30, 2020 was obtained retrospectively from their electronic medical records and analyzed. Data of the patients who underwent the COVID-19 test and data of the patients allocated to emergency department isolation beds were analyzed.Results: A total of 1,409 patients who visited emergency department based screening clinic were included in the study. Among them 1378 patients underwent the COVID-19 test. 1226 patients were allowed to go home after the COVID-19 test, and 152 patients were allocated to the emergency department isolation beds. In statistical analysis, affecting allocation of emergency department isolation beds factors were old age, history of overseas travel within 14 days, fever. In addition, statistical analysis confirmed that emergency department isolation beds were properly allocated. Conclusion: When COVID-19 suspected patient visits an emergency department based screening clinic, the allocation of emergency department isolation beds should be considered for patients who are old age, have a history of overseas travel within the last 14 days, and have fever.
