Abstract
The Japanese Society for Dialysis Therapy reported that a total of 7050 patients had been on dialysis for more than 10 years at the end of 1985. We obtained clincal data on 2065 of these patients from 92 institutions. The survey showed: A majority of the patients (79.5%) had suffered originally from chronic glomerulonephritis and within 5 years they developed renal failure. They started dialysis therapy one year after renal failure. Among these patients 5% are now hospitalized, 50.8% are visiting dialysis clinics in the daytime and 43.1% at night. Most of these patients (97.6%) are receiving hemodialysis three times a week for a total of 13 hours and a minority (1.1%) receive CAPD.Of all patients, 14.5% had blood transfusions within the last year. And the average volume of blood was 1428ml. Of these patients 32.4% developed serious complications and 23.2% are still suffering from the complications. The major complication was renal osteodystrophy: Calcification of vessels and soft tissues appeared in 15% of the patients, bone fracture in 6.0% and up to 36.0% of the patients had pain in bones and joints. Some (7.3%) of the patients already had parathyroidectomy. Notably, carpal tunnel syndrome was seen in 13.1% of the patients.At present 71.2% of the patients, including house wives, are fully rehabilitated, working at least 5 days a week, and 60% or more of these patients are satisfied with their life conditions. Although some of them have suffered from various complications, they claim that their quality of life is satisfactory even after 10 years of dialysis.Analyses of data from clincal laboratory tests on the patients revealed that we are successful in keeping their blood pressure at 129-74mmHg, Ht at 26.8%, predialysis BUN at 81.6mg/dl and Cr at 13.2mg/dl.
