Abstract
A 73-year-old man presented with lumbar pain and reduced vision. He was diagnosed as having a liver abscess, endophthalmitis and sepsis. Klebsiella pneumoniae was detected in the abscess following percutaneous drainage. The patient was diagnosed as having lumbar vertebral body inflammation and an epidural abscess based on imaging examinations. The patient's condition improved with conservative therapy and he was transferred to another hospital at the 52nd day after his arrival. According to the 10 reported cases of invasive K. pneumoniae syndrome with a liver abscess in Japan, 7 cases (70%) were caused by endophthalmitis and 2 cases (20%) died. When we come across patients with a liver abscess and endophthalmitis, it is important to suspect invasive K. pneumoniae syndrome. Furthermore, we should start treatment with antibacterial drugs taking metastatic lesions into consideration and perform multimodal therapy including surgery.
