Abstract
We examined the prognostic factors in patients with peritonitis caused by perforation of the left hemicolon who underwent emergency surgery at our hospital. The subjects were 59 patients aged 78 years old(median)and 10(16.9%)of the patients died. Significant poor prognostic factors included preoperative shock, preoperative leukopenia, preoperative base excess(BE), and preoperative sequential organ failure assessment(SOFA)score. Direct hemoperfusion with polymyxin B-immobilized fiber(PMX-DHP)was performed, especially for serious patients. Subgroup analysis of the above 4 poor prognostic factor groups and the Hinchey stage Ⅳ group was performed to evaluate whether PMX-DHP contributed towards the prognosis. We could not show any beneficial effect on the prognosis of PMX-DHP in any of the subgroups(groups with preoperative shock, preoperative leukopenia, lower preoperative BE, higher preoperative SOFA score, and Hinchey stage Ⅳ).
