Abstract
Two cases of breast-conserving surgery to treat breast cancer in which breast reconstruction was performed simultaneously using endoscopy-assisted breast-conserving surgery and free dermal fat grafting in order to preserve the postoperative esthetic outcome are reported. Case 1 was a 35-year-old woman who had a 2-cm tumor in the upper outer quadrant of the right breast and axillary lymph node swelling. After performing axillary lymph node dissection, the endoscope was inserted from the axillary wound, and the anterior surface of the pectoralis major muscle was detached. With an areolar incision and construction of a skin flap, partial mastectomy was performed. Free dermal fat tissues for grafting were taken from the right lower abdomen and transplanted to the defective area. Chemoradiotherapy was performed postoperatively. Case 2 was a 52-year-old woman who had a 1.8-cm tumor in the upper outer quadrant of the left breast. Sentinel lymph node biopsy was performed. The same surgery as that for Case 1 was performed, and radiation and hormone therapies were performed postoperatively. Both cases achieved a favorable esthetic outcome, and this procedure appears to be a new appeoach to improve esthetic outcomes after breast-conserving surgery to treat breast cancer.
