Abstract
We report a case of caesarean section for a pregnant woman with COVID-19 after sufficient simulation. A 29-year-old pregnant Japanese woman(gravida 1 para 1)at 37-week gestation was admitted to our hospital with positive SARS-CoV-2 PCR results. She complained of mild fatigue, dysgeusia, and dysosmia. We performed caesarean section with sufficient preparations such as zoning, selecting appropriate personal protective equipment (PPE)for surgery, training for PPE donning and doffing, and a simulation by the entire hospital. We chose to induce spinal anesthesia without epidural anesthesia. After SARS-CoV-2 PCR test results were confirmed to be negative for two successive days, the patient was discharged on the 19th postoperative day without any complications. SARS-CoV-2 PCR test results were negative for the baby, and no nosocomial infections also occurred. During a pandemic, we should be able to promptly respond to unknown infections such as COVID-19 that do not have established guidelines. In this case, we were able to treat the patient safely due to sufficient training, simulation, and the creation of a manual. It is essential to establish formal protocols, undergo sufficient training and perform simulations. To accomplish this, we need to form a multidisciplinary team that involves cooperation from the entire hospital.
