Abstract
Oncoplastic surgery using pedicled perforator flap is an OPBCS volume replacement level 2 procedure. This paper focuses on the chest wall perforator flap (CWPF) in the lateral thoracic region, represented by the thoracodorsal artery perforation flap. This procedure minimizes donor site morbidity and results in less postoperative flap volume loss than latissimus dorsi myocutaneous flap. On the other hand, it requires careful dissection of the perforator and handling with the flap. The indication for lateral CWPF is mainly patients with defects in the outer quadrants up to 25% of the total breast volume. In CWPF patients, isolation of the descending branch of the thoracodorsal artery allows the flap to reach more distant defects. Preoperative detection of reliable perforators is important for a safe procedure, and color Doppler sonography in lateral decubitus position, the same position used in the operation is useful. This article provides an overview of the lateral CWPF including preoperative design, intraoperative technique, tips, and pitfalls.
