Atherothrombotic brain infarction developed in basilar artery fenestration: a case report / 脳底動脈の窓形成部に生じたアテローム血栓性脳梗塞の1 例

Abstract
We reported a 74-year-old man with atherothrombotic brain infarction developed in basilar artery fenestration complaining of ataxic gait and scanning speech. He was suffering from hypertension, diabetes mellitus, and dyslipidemia. MRI revealed subacute infarction in right middle cerebellar peduncle. 3D CTA showed fenestration in the proximal portion of the basilar artery with occlusion of right arterial trunk and right anterior inferior cerebellar artery. Intracranial plaque imaging was useful for visualizing vulnerable plaque in occluded trunk. Previously described cases of atherothrombotic cerebral infarction in the basilar artery fenestration tend to be observed in elderly person with risk factors for stroke such as hypertension, dyslipidemia, and diabetes mellitus. Complex local hemodynamics and risk factors for stroke could mutually accelerate plaque formation causing cerebral infarction.
