TWO CASES OF HEPATECTOMY FOR HEPATOCELLULAR CARCINOMA INCLUDING BILOMA FORMED AFTER TRANSCATHETER ARTERIAL EMBOLIZATION

Abstract
We report two cases of hepatectomy for biloma formed after transcatheter arterial embolization (TAE) for hepatocellular carcinoma (HCC). A 67-year-old man undergone TAE for HCC with a background factor of chronic alcoholic liver disease was diagnosed as having biloma. Thereafter recurrence of HCC in the liver S8 was suspected. Another patient, a 65-year-old woman undergone TAE for HCC with a background factor of hepatitis type C was found to have multiple bilomas 1.5 months after the TAE. Thereafter, tumor markers raised again and recurrence of HCC was suspected. But differential diagnosis between HCC and biloma was difficult on imaging. In both patients, hepatectomy for HCC including bilomas was performed. We think that surgical resection including cancer foci must be kept in mind, if bilomas do not respond to conservative therapy, or biloma remnant severely limits later treatment of cancer.
