Abstract
We herein report a case of lower rectal cancer complicated with idiopathic interstitial pneumonia which had a poor prognosis (3-year survival rate : 50%). We chose preoperative chemoradiation therapy : 5FU 600mg/body×5 days (d1-5, intravenous), CDDP 10mg/body×5 days (d1-5, intravenous), pelvic irradiation 40Gy in 20 fractions over 4 weeks. The tumor was resected by transanal endoscopic microsurgery (TEM) 5 weeks after the completion of the chemoradiation therapy. A histopathological study of the resected specimen showed well differentiated adenocarcinoma with submucosal invasion and the surgical margin was free of cancer. A year after the operation, computed tomography study showed no recurrences or metastasis. We concluded that preoperative chemoradiation therapy with a local resection of the tumor, such as TEM, may thus be an effective therapeutic alternative in some T1 and T2 cases of rectal cancer.
