Abstract
A 73-year-old man with a 30-year history of anal fistula was diagnosed with a perianal tumor. The tumor had developed at the 3-7 o'clock position in the anal canal. Furthermore, rectal cancer was observed via colonoscopy. Histologically, both the rectal and perianal cancers were identified as well differentiated adenocarcinomas, and we suspected that the anal fistula cancer had metastasized from the rectal cancer. The patient underwent a minimally invasive laparoscopic abdominoperineal radical resection, and we confirmed that the anal fistula cancer showed the same appearance as the rectal cancer via immunohistological analysis. Consequently, we diagnosed this case as rectal cancer with metastatic anal fistula cancer.
