A Case of Ileal Mesenteric Castleman’s Disease and a Review of 57 Japanese Cases of Castleman’s Disease Affecting the Abdominal Mesentery / 回腸間膜に発生したCastleman病の1例―本邦腹腔内発生報告57例の検討―

Abstract
A 40-years-old woman was diagnosed with abnormal CA19-9 levels while she was under observation for myoma uteri and an ovarian cyst. Colonoscopy revealed pressure caused by extra-lumen formation in the Bauhin’s valve. Enhanced computed tomography revealed a hyper-vascular tumor in the ileo-cecal portion, which was supplied by the ileo-cecal artery and was 35×25mm in size. Magnetic resonance imaging showed a tumor with low signal intensity on T1-weighted images, and iso-signal intensity on T2-weighted images. We could not rule out the possibility of a malignant tumor, so we decided to perform surgery. We resected the tumor that was situated in the ileal mesentery through a small incision at the navel. Histologically, the tumor was found to be a lymph node, with an increased number of lymphoid follicles with small germinal centers. The lymphoid follicles were surrounded by a broad mantle zone. Sclerotic blood vessels penetrated the germinal centers. The patient was diagnosed with unicentric Castleman’s disease ( hyaline-vascular type ). She has no relapse for 1 year and 8 months since the operation. In addition, we reviewed Japanese cases of Castleman’s disease affecting the abdominal area.
