Abstract
We report a rare case of ischemic colitis associated with inferior mesenteric arteriovenous malformation (AVM). To the best of our knowledge this is the first case to be reported in the Japanese literature. A 74-year-old man was referred to our hospital because of hematochezia. Colonoscopy revealed severe mucosal edema and congestion in the left colon, suggesting ischemic colitis. A contrast enhanced CT showed a poor enhancement of the left colon, with two inferior mesenteric AVMs, which were confirmed by selective inferior mesenteric arteriography. We failed to treat it by endovascular embolization, because of acute exacerbation of colonic edema during the procedure. A left hemicolectomy was then performed with an end transverse colostomy. The post-operative course was uneventful and intestinal bleeding has not been observed at 7 months after surgery.
