We report a case of pathological CR after Imatinib mesylate therapy in a rectal GIST. A patient was a 76-year-old man who underwent lung cancer surgery. Follow-up PET-CT for the lung cancer showed FDG uptake in the rectum. FNA revealed rectal GIST. The tumor existed on the anterior wall of the Rb rectum, and widely bordered on the prostate gland, so we performed Imatinib mesylate therapy for the neoadjuvant chemotherapy. The tumor shrank from 28mm to 11mm in diameter, so we performed surgery. Pathological findings showed complete response for the chemotherapy. During 84 months of monitoring, there is no evidence of recurrence.
