Psicosomatica e dermopatie: dati preliminari sulle correlazioni con alessitimia, meccanismi di difesa, personalita e qualita di vita

Abstract
Key Words: psoriasis, vitiligo, alexitymia, personality Objective: Some studies on the onset and clinical history of some dermatological diseases as psoriasis showed a relevant coincidences with emotional stress and psychic conflicts, supporting the hypothesis of a tight relationship between cutaneous alterations and deficit in the individual process of psychological development. According to the data in the medical literature, we aim to investigate these aspects and overall the correlation with personality, defense mechanisms, alexithymia, quality of life and some dermatological diseases such as psoriasis and vitiligo. Method: The study group is composed of 100 people: 50 patients (29 affected by vitiligo and 21 by psoriasis) and 50 healthy controls. We have done a clinical-anamnestic assessment and we administrated the following tests: Defense Mechanism Inventory (DMI), Eysenck Personality Questionnaire (EPQ-R Short Version), IVE (Impulsiveness Questionnaire), Toronto Alexithymia Scale (Tas- 20), SF 36 Health Survey. Results: The analysis of the personality profiles investigated with Eysenck Personality Questionnaire (EPQ-R Short Version) and IVE (Impulsiveness Questionnaire) showed statistically significant higher scores in neuroticism (p=0.02) and empathy (p=0.004) among cases and statistically significant higher scores in extraversion (p= 0,005) among controls. Analyzing the total average score in the test TAS-20 we found a statistically significant difference between cases and controls, who had respectively a score of 51.38 and 47.34. In the group of the patients with dermatological diseases the score is significantly higher (p=0,04) than healthy controls, showing a higher level of alexithymia. Conclusions: Although genetic-constitutional factors predispose to the onset of vitiligo and psoriasis, we can suppose, according to our results, the contribution of psycho-environmental factors on the development of these diseases and above all the presence of a primitive deficit in the mentalization process and of the emotions elaboration.
