Abstract
Úvod: Většinu karcinomů prsu je možné odstranit prs šetřící operací – parciální resekcí. Tato operace doplněná adjuvantní radioterapií je onkologicky stejně účinná jako mastektomie, četnost lokálních recidiv je také srovnatelná. Pro přijatelný kosmetický efekt je snaha odstranit nádor jen s malým množstvím okolní žlázy. Materiál a metodika: Cílem našeho sdělení bylo zjistit význam pozitivity resekčních okrajů na vznik lokální recidivy karcinomu prsu a vliv na celkové přežití nemocných. Jednalo se o soubor 183 pacientek, u kterých byla v letech 2006–2008 provedena prs šetřící operace pro karcinom. Výsledky: 21 pacientek z našeho souboru nemělo provedenou reresekci při pozitivním resekčním okraji. Všechny podstoupily adjuvantní radioterapii a systémovou léčbu. Žádná dosud neměla lokální recidivu. 17 pacientek žije, 2 zemřely na generalizaci onemocnění, 2 zemřely na jiné onemocnění. Pětileté přežití je 86 %, pětiletý interval bez nemoci 89 %. Závěr: V našem souboru nebyl výskyt lokální recidivy při pozitivním resekčním okraji do 5 let po operaci pozorován. Přístup ke každé pacientce musí být individuální.

This operation supplemented with adjuvant therapy is as oncologically efficient as mastectomy, the number of local recurrences is similar, too. Due to acceptable cosmetic effects, there is an effort to remove the tumor only with a small amount of circumjacent gland. Material and methodology: The aim of our report was to find out the influence of positive resection margins on local recurrence of breast cancer and overall survival. In the group, there were 183 patients breast conserving surgery had been performed for carcinoma in from 2006 to 2008. Results: Resection was not performed in 21 patients of our group where resection margins were positive. All of these underwent adjuvant radiotherapy and systemic treatment. So far no patient has had local recurrence. 17 patients are alive, two of them died of the generalization of disease and two patients died of another disease. The five-year survival rate is 86 % and the disease-free interval of five years is 89 %. Conclusion: In our group, there was no local recurrence within five years after surgery if the resection margin was positive. The approach to each patient has to be individual.
