Abstract
患者男,84岁,面部褐色丘疹、斑块3年,泛发全身伴瘙痒2个月.皮肤科情况:全身多发圆形、不规则褐色丘疹、斑块,境界清楚,表面呈颗粒状、疣状或上覆油腻性鳞屑,无糜烂、渗出.皮损组织病理示:角化过度,棘层肥厚,呈乳头瘤样增生,假性角囊肿基底位于同一水平面上.肺部病变组织病理示:右肺上叶周围型腺癌.诊断:Leser-Tré1at征;右上肺腺癌(cT1cN0M0).予以手术切除肺部病变,随访中.
The 84-year-old male patient had 3 years of brown facial papules and plaques and generalized itching for 2 months.Dermatological examination showed round and irregular brown papules and plaques with clear boundaries on the whole body.The surface was granular,verrucous or covered with greasy scales.No erosion and oozing were observed.Lesion histopathology showed hyperkeratosis,acanthosis,papillomatoid hyperplasia,and the pseudokeratocyst on the same horizontal plane.Pathology of lung lesions showed peripheral adenocarcinoma of the right upper lobe.Diagnosis:Leser-Trelat sign with right upper lung adenocarcinoma(cT1cNOMO).The pulmonary lesions were surgically resected and followed up.
