Abstract
目的 分析血清富含脯氨酸蛋白14(PRR14)在结直肠息肉(CRP)与结直肠癌(CRC)中的表达,及其与CRC预后的相关性.方法 回顾性分析2018年1月-2022年10月在西安交通大学医学院第一附属医院首次诊断为Ⅰ~Ⅲ期的70例CRC患者、同期70例CRP患者以及85例健康志愿者的临床资料.使用酶联免疫吸附试验检测血清PRR14水平.随访CRC患者总生存期.结果 CRP患者及CRC患者的血清PRR14水平显著高于健康志愿者,CRC患者升高最明显,CRP患者次之(P<0.001).经受试者操作特征曲线分析,血清PRR14及肿瘤标志物联合鉴别诊断CRP与CRC的曲线下面积为0.910(95%CI:[0.863,0.957]),显著提高了肿瘤标志物鉴别诊断效能.血清PRR14水平>3.67 ng/mL的CRC患者淋巴结转移数更多,T分期、TNM分期更高(P<0.05).Kaplan-Meier生存曲线分析显示,血清PRR14高水平亚组的中位生存期显著低于低水平亚组(χ2=30.799,P<0.001).经单因素和多因素Cox回归分析,血清PRR14水平>3.67 ng/mL是CRC患者总生存期的独立危险因素(P<0.05).结论 血清PRR14可提高常规血清肿瘤标志物鉴别CRP及CRC的效能,且血清PRR14高水平与CRC患者预后不良有关,可作为CRC诊断和预后的有效生物标志物.
Objective To analyze the expression of serum proline-rich protein 14(PRR14)in colorectal polyp(CRP)and colorectal cancer(CRC),and its correlation with the prognosis of CRC.Methods The clinical data of 70 patients with CRC first diagnosed with stageⅠto Ⅲ in The First Affiliated Hospital of Xi'an Jiao Tong University from January 2018 to October 2022,and 70 patients with CRP and 85 healthy volunteers during the same period were retrospectively analyzed.Serum PRR14 levels were detected via enzyme-linked immunosor-bent assay.The overall survival of CRC patients were recorded during follow-up.Results The serum PRR14 level in CRP patients and CRC patients was significantly higher than that in healthy volunteers,and the increase was most obvious in CRC patients,and patients with CRP being followed(P<0.001).According to the receiver operator characteristic curve,the area under the curve of serum PRR14 combined with tumor markers in the differential diagnosis of CRP and CRC was 0.910(95%CI:[0.863,0.957]),which significantly improved the differential diagnosis efficiency of tumor markers.CRC patients with a serum PRR14 level>3.67 ng/mL had more lymph node metastasis and a higher T stage and TNM stage(P<0.05).Kaplan-Meier survival curve analysis showed that the median overall survival stage of the high level of PRR14 subgroup was significantly lower than that of the low level subgroup(χ2 = 30.799,P<0.001).By univariate and multivar-iate Cox regression analysis,the serum PRR14 level>3.67 ng/mL was still an independent risk factor for over-all suruival in CRC patients(P<0.001).Conclusion Serum PRR14 can improve the efficacy of routine serum tumor markers in differentiating CRP and CRC,and a high serum PRR14 level is related to poor prognosis of CRC patients,so this indicator can be used as an effective biomarker for diagnosis and prognosis of CRC.
