Abstract
背景:糖酵解功能与结直肠癌的增殖、转移、耐药性等具有明显相关性,对如何量化评估糖酵解水平还缺乏相应指标.目的:探讨术前 18F-FDG-PET/CT糖代谢参数与结直肠癌患者临床病理特征、B7-H3、CD8+ T细胞表达的相关性.方法:收集2021年4月—2022年7月苏州大学附属第一医院确诊的40例结直肠腺癌患者,所有患者在术前2周内行全身 18F-FDG-PET/CT检查,获取最大标准化摄取值(SUVmax)、平均标准化摄取值(SUVmean)、肿瘤代谢体积(MTV)、病灶糖酵解总量(TLG).采用免疫组化染色检测B7-H3、CD8+ T细胞表达,并分析其与临床病理特征、18F-FDG-PET/CT糖代谢参数的相关性.结果:MTV、TLG与结直肠癌患者的T分期、淋巴结转移相关(P<0.05),而SUVmax、SUVmean、MTV、TLG与其他临床病理特征无关(P>0.05).与癌旁组织相比,结直肠癌组织中B7-H3表达显著升高(P<0.001),CD8+ T细胞表达显著降低(P<0.001).结直肠癌组织中B7-H3表达与CD8+ T细胞表达无关(P>0.05);B7-H3表达与T分期、淋巴结转移相关(P<0.05);CD8+ T细胞表达与肿瘤大小、分化程度相关(P<0.05).结直肠癌组织中B7-H3表达与SUVmax、SUVmean、MTV和TLG相关(P<0.05),CD8+ T细胞与MTV、TLG相关(P<0.01).结论:18F-FDG-PET/CT糖代谢参数中的MTV和TLG能更有效地反映结直肠癌病理特征和转移情况,可能具有更高的评估价值.在结直肠癌组织中,B7-H3明显高表达、CD8+ T细胞明显低表达,两者与PET/CT糖代谢参数均存在相关性.联合 18F-FDG-PET/CT和B7-H3、CD8+ T细胞的表达情况可能为结直肠癌临床判断分期、转移以及评估糖酵解水平等提供更为可靠的参考价值.
Background:Glycolytic function is obviously related to the proliferation,metastasis and drug resistance of colorectal cancer,and there is still a lacking of corresponding indicators for quantitatively evaluating the level of glycolysis.Aims:To investigate the correlation between 18F-FDG-PET/CT glucose metabolism parameters and clinicopathological features,as well as the expressions of B7-H3 and CD8+ T cells in colorectal cancer patients.Methods:Forty patients with colorectal adenocarcinoma from April 2021 to July 2022 at the First Affiliated Hospital of Soochow University were recruited.All patients underwent whole-body 18F-FDG-PET/CT examination within 2 weeks before surgery,the maximum standardized uptake value(SUVmax),average standardized uptake value(SUVmean),metabolic tumor volume(MTV),and total lesion glycolysis(TLG)were measured.Expressions of B7-H3 and CD8+ T cells were determined by immunohistochemistry,and their correlation with clinicopathological features,18F-FDG-PET/CT glucose metabolism parameters were analyzed.Results:MTV,TLG were correlated with T staging and lymph node metastasis in colorectal cancer patients(P<0.05),however,SUVmax,SUVmean,MTV,TLG were not correlated with other clinicopathological features(P>0.05).Compared with paracancerous tissue,expression of B7-H3 in colorectal cancer tissue was significantly increased(P<0.001),while expression of CD8+ T cells was significantly decreased(P<0.001).Expression of B7-H3 was not correlated with the expression of CD8+ T cells in colorectal cancer tissue(P>0.05).Expression of B7-H3 was correlated with T staging,lymph node metastasis(P<0.05).Expression of CD8+ T cells in colorectal cancer tissue was correlated with tumor size and differentiation degree(P<0.05).There was a significant correlation between the expression of B7-H3 in colorectal cancer tissue and SUVmax,SUVmean,MTV,and TLG(P<0.05),while expression of CD8+ T cells was correlated with MTV,TLG(P<0.01).Conclusions:MTV and TLG in 18F-FDG-PET/CT glucose metabolism parameters can effectively demonstrate the pathological features and metastasis of colorectal cancer,and may have higher evaluation value.In colorectal cancer tissue,B7-H3 is significantly overexpressed and CD8+ T cells are significantly underexpressed,both of which are correlated with PET/CT glucose metabolism parameters.18F-FDG-PET/CT combined with expressions of B7-H3 and CD8+ T cells may provide more reliable reference value for clinical staging,metastasis,and evaluation of glycolysis levels in colorectal cancer.
