Abstract
目的 探讨环氧合酶-2(COX-2)、胃泌素17(G-17)、miR-92a表达及幽门螺杆菌(Hp)感染与结直肠息肉、结直肠癌发生的相关性.方法 选取2021年1月至2022年12月在四川绵阳四○四医院就诊的结直肠息肉患者62例(息肉组),结直肠癌患者48例(肿瘤组),检查两组组织COX-2、外周血G-17、粪便miR-92a表达以及幽门螺旋杆菌感染情况,分析其与结直肠息肉、结直肠癌发生的相关性.结果 肿瘤组COX-2阳性表达率和Hp感染率分别为68.75%和75.00%,明显高于息肉组(P<0.05);肿瘤组G-17和miR-92a相对表达量分别为(25.56±7.76)pmol/L和(1.94±0.43),明显高于息肉组(P<0.05);息肉最大直径≥10mm患者COX-2阳性表达率、G-17、miR-92a相对表达量和Hp感染率分别为41.94%、(22.42±5.28)pmol/L、(1.29±0.25)和38.71%,明显高于息肉最大直径<10mm患者(P<0.05);息肉多发患者COX-2阳性表达率、G-17、miR-92a相对表达量和Hp感染率分别为45.83%、(23.12±5.14)pmol/L、(1.33±0.23)和54.17%,明显高于息肉单发患者(P<0.05);多发肿瘤患者G-17和miR-92a相对表达量分别为(30.72±6.00)pmol/L和(2.12±0.33),明显高于单发肿瘤患者(P<0.05);TNM分期Ⅲ~Ⅳ期患者COX-2阳性表达率、G-17、miR-92a相对表达量和Hp感染率分别为100.00%、(29.60±6.80)pmol/L、(2.15±0.44)和100.00%,明显高于Ⅰ~Ⅱ期患者(P<0.05);息肉组Hp感染患者COX-2阳性表达率、G-17和miR-92a相对变量分别为56.25%、(23.42±5.65)pmol/L和(1.34±0.21),明显高于无Hp感染患者(P<0.05);肿瘤组Hp感染患者COX-2阳性表达率、G-17和miR-92a相对变量分别为83.33%、(27.12±5.109)pmol/L和(2.12±0.26),明显高于无Hp感染患者(P<0.05).结论 COX-2、G-17、miR-92a表达及Hp感染与结直肠息肉、结直肠癌临床特征有关,同时Hp感染可提高COX-2、G-17、miR-92a 表达.
Objective To investigate the relationship between the expression of cyclooxygenase-2(COX-2),Gastrin 17(G-17),miR-92a,Helicobacter pylori(Hp)infection and colorectal polyps and cancer.Methods A total of 62 patients with colorectal polyps(polyp group)and 48 patients with colorectal cancer(tumor group)were selected from our hospital from January 2021 to December 2022.The expression of COX-2,peripheral blood G-17 and fecal miR-92a and helicobacter pylori infection in the tissues of the two groups were examined,and their correlation with colorectal polyps and colorectal cancer was analyzed.Results The positive expression rate of COX-2 and Hp infection rate in the tumor group were 68.75%and 75.00%,respectively,which were significantly higher than those in the polyp group(P<0.05).The G-17 and miR-92a relative expression levels in the tumor group were(25.56±7.76)pmol/L and(1.94±0.43),respectively,whichwassignificantly higher than those in the polyp group(P<0.05).The positive expression rate of COX-2,G-17,relative expression levels of miR-92a,and Hp infection rate in patients with a maximum diameter of polyps ≥10 mm were 41.94%,(22.42±5.28)pmol/L,(1.29±0.25),and 38.71%,respectively,which were significantly higher than those in patients with a maximum diameter of polyps<10 mm(P<0.05).The positive expression rate of COX-2,G-17,relative expression levels of miR-92a,and Hp infection rate in patients with multiple polyps were 45.83%,(23.12±5.14)pmol/L,(1.33±0.23),and 54.17%,respectively,which were significantly higher than those in patients with single polyps(P<0.05).The G-17 and relative expression levels of miR-92a in patients with multiple tumors were(30.72±6.00)pmol/L and(2.12±0.33),respectively,which were significantly higher than those in patients with single tumors(P<0.05).The positive expression rate of COX-2,G-17,relative expression levels of miR-92a,and Hp infection rate in TNM stage Ⅲ-Ⅳ patients were 100.00%,(29.60±6.80)pmol/L,(2.15± 0.44),and 100.00%,respectively,which were significantly higher than those in stage Ⅰ-Ⅱ patients(P<0.05).The positive expression rate of COX-2,G-17 and relative expression levels of miR-92a in patients with Hp infection in the polyp group were 56.25%,(23.42±5.65)pmol/L,and(1.34±0.21),respectively,which were significantly higher than those in patients without Hp infection(P<0.05).The positive expression rate of COX-2,G-17 and relative expression levels of miR-92a in Hp infected patients in the tumor group were 83.33%,(27.12±5.109)pmol/L,and(2.12±0.26),respectively,which were significantly higher than those in patients without Hp infection(P<0.05).Conclusion The expression of COX-2,G-17,miR-92a and Hp infection are related to the clinical characteristics of colorectal polyps and colorectal cancer,while Hp infection can increase the expression of COX-2,G-17,miR-92a.
