安宁疗护患者疼痛管理中的风险及应对策略 / Risks and Strategies in Pain Management for Hospice Patients

Abstract
在对相关文献进行系统梳理的基础上,系统阐述了安宁疗护疼痛管理中存在的阿片类药物使用不足、过量使用或滥用,以及错误使用等问题;呼吸抑制、成瘾、谵妄、物质使用不当、镇静、跌倒受伤与机动车事故等风险;使用的风险评估与筛查工具;以及不同类型风险的管理策略:根据主体风险态度进行管理,利用综合疼痛管理应对成瘾风险,通过监督与筛查应对滥用风险,采用指南与培训应对使用不当风险,利用药物监测与轮换应对谵妄风险,通过早期识别应对呼吸抑制风险.
Based on a systematic review of relevant literature,the current situation of pain management in hospice care was systematically elaborated,including insufficient use,excessive or overuse,and misuse of opioid drugs.It discusses the risks associated with pain management,including respiratory suppression,addiction,delirium,improper substance use,sedation,fall injuries,and motor vehicle accidents.The paper also explores risk assessment and screening tools and outlines management strategies for different types of risks.These strategies encompass a patient-centered approach to risk management,the use of comprehensive pain management to address addiction risk,monitoring and screening to address abuse risk,guidelines and training to manage inappropriate use risks,drug monitoring and rotation to address delirium risk,and early identification to manage respiratory depression risks.
