红细胞分布宽度与冠心病及其冠状动脉病变程度的相关性研究 / Study on the correlation between RDW and coronary heart disease and extent of coronary atherosclerosis

Abstract
目的 分析红细胞体积分布宽度(red blood cell distribution width,RDW)与冠心病(coronary heart disease,CHD)及冠状动脉病变Gensini积分之间的关系.方法 选取自2012年1月—2014年6月于北京市垂杨柳医院心内科及心脏重症监护室院的患者行冠状动脉造影检查的513例疑诊冠心病患者作为研究对象,根据冠脉造影结果分为两组,冠心病组416例,对照组97例.选用多元Logistic回归分析冠心病的危险因素,探讨RDW与冠心病的相关性;采用冠状动脉Gensini积分评价冠脉病变的严重程度,双变量Spearman相关分析法分析Gensini积分、血白细胞计数、血清超敏C反应蛋白与RDW的关系.结果 冠心病组RDW水平高于对照组,差异有显著性(P<0.05);二元Logistic回归分析显示,RDW为冠心病发生的独立危险因素(r=1.012,OR=2.751,P<0.05).双变量Spearman相关分析法分析显示RDW水平与冠状动脉病变严重程度Gensini积分正相关(r=0.294,P<0.05),显著正相关.结论 RDW与冠心病及冠状动脉病变严重程度相关,可能作为冠心病及冠状动脉病变严重程度的预测因素.
Objective The aim of this study was to use Gensini score for the evaluation of the severity of coronary artery disease in patients undergoing coronary angiography, and to observe the relationship between the level of RDW and the incidence of coronary heart disease and coronary artery lesions of Gensini score.Method416 cases of patients with coronary heart disease were selected from the Department of Cardiology and Cardiovascular care unit of Beijing ChuiYangLiu Hospital in hospitalized patients diagnosed by coronary angiography from 2011 Jan to 2012 June. The normal control group: 97 patients with negative coronary angiography results were selected as the normal control group.Result With the worse of the severity of coronary artery stenosis, the level of RDW is increased. There was a significant linear correlation between the RDW and Gensini score.The level of RDW are closely related with the stability of coronary heart disease, the clinical type of CHD groups is more unstable, the level of RDW is more higher.Conclusion The level of RDW are closely related with the pathogenesis of coronary heart disease, logistic regression analysis showed, in effect correction factors, the level of RDW remained an independent predictive factor of CHD.
