Abstract
Cheiro-pedal syndrome (CPS) is an incomplete sensory disorder characterized by sensory disturbances in the hand and foot on the same side. The underlying cause of the syndrome varies, but lacunar infarction is considered the most common cause. The responsible lesions could be anywhere from the brain parenchyma to the cervical spinal cord. However, the syndrome is rarely reported and can be easily mistaken for peripheral neuropathy. We present a case of CPS in a 64-year-old man with sudden onset of paraesthesia in a small region of the right hand and right foot. Magnetic resonance imaging of the brain revealed an acute infarction in the right pons region. In addition, further investigation of the etiology of ischemic stroke revealed a thrombus at the apex of the left ventricle identified by transthoracic echocardiography scan. Due to the mild symptoms of discontinuous sensory disturbance, the syndrome is often misdiagnosed in outpatient clinics or emergency departments, resulting in delays in treatment. By reporting such cases, clinicians will be more aware of these rare syndromes. A complete diagnostic examination must be performed, as the main cause of CPS is believed to be a lacunar infarction. Early diagnosis of stroke etiology can help with appropriate treatment that can effectively prevent stroke recurrence.
