Objective: Population aging is a global trend. The problems that come with aging are chronic diseases, and medical needs have thus become an essential part of long-term care. We attempted to find the public touched indices from the integrated care experience in the Yongchang community in Zhongzheng District of Taipei City under the treatment-maintenance care ideation. Method: Descriptive statistical analysis was performed to analyze the demographics data collected from 94 people at the service station of the Yongchang community. We also analyze nine indices to find the feasible ones that can be achieved, which are keys to achievement of the care project use using the Pearson correlation tests. Result: Among the nine key indices, the top three with the highest effectiveness are 'Achieving Delayed Disability' (38.3%), followed by reducing the burden of caregivers (36.1%), and improvement of depression (33%). Conclusion: The health care model at the service station of Yongchang community has integrated home health care with community care, and reduced the burden of caregivers, delayed disability, improved depression in disabled elderly, and provided strong supports for their families.
