Pioneering of Multicultural Medical Education in Medical Humanity: Development and Validation of Taiwan Medical Cultural Competence Scale

Abstract
Purpose: Medical humanities education is cultivating cultural competence for medical students to deliver healthcare to patients with diverse backgrounds in the future. However, it lacks a radical approach to patient empowerment in a multicultural context. This study was conducted to create and validate a Taiwan Multicultural Competence Scale (TMCC) to evaluate the impact of multicultural education on medical students. Methods: The research comprised three stages: first, creating a 15-question TMCC scale with four aspects; second, refining the scale based on expert feedback, introducing options like 'suitable' and 'delete'; and third, implementing a pre-test scale validated by experts specializing in multiculturalism and medical professionalism education, this study is a purposive sampling method, 151 outpatient patients were recruited from Jan. to Mar. 2023. Results: Item analysis showed that the test of item homogeneity was significantly corrected (> .40). Exploratory factor analysis (EFA) identified one factor explaining over 26%, 24%, 14%, and 18% of the variation in awareness, skill, knowledge, and relationship constructs, respectively. Reliability analysis indicated high internal consistency (Cronbach's α > 0.7). Pearson correlation analysis revealed positive correlations between awareness, skill, knowledge, and relationship constructs, suggesting that higher TMCC perception scores correlated with elevated TMCC skills, cognition, and relationship scores. The study met recommendations for internal consistency, reliability, and convergent construct validity in developing the TMCC scale. Conclusions: Our results suggested that the TMCC scale had good reliability and validity and could be used to assess the cultural competency of medical students.
