Abstract
The article discusses efforts made by Cleveland Clinic Health System (CCHS) to improve clinical documentation improvement and quality of care by innovation and transformation. Some of these include electronic health records (EHRs), pay for performance, and The International Classification of Diseases, 10th revision, Clinical Modification/ Procedure Coding System (ICD-10-CM/PCS). The use of computer-assisted coding (CAC) to improve, coding, data extraction, and patient care is described.
