Abstract
The article focuses on transitions out of institutional settings such as inpatient mental health care, residential substance use services, out-of-home care (OHC), and corrections that are associated with significant risks of housing insecurity which can be greatly reduced by more effective service coordination. Topics include examines that effective service coordination is especially critical between separate service systems.
