Abstract
The article presents a study aimed at evaluating the efficacy of a multidisciplinary diabetes self-management program. It notes that a group of 44 type 2 diabetes patients with HbA1c levels greater than 9.5 over a 12-month period was identified and 23 were enrolled in a synchronous care group. Results revealed the synchronous management approach improved HbA1c level over standard management for indigent Hispanic patients with poorly controlled type 2 diabetes.
