Abstract
The article discusses the fundamental issues in the management of Barrett's oesophagus (BO) and to gain insight on the challenges faced by the BO population in South Africa. It mentions that the American Gastroenterology Association (AGA) advocates three pillas of management to combat BO which are acid suppression, surveillance, and treatment of dysplasia. It notes that the clinical goal of acid suppression is the reversion of Barrett's metaplastic epithelium and the diminution of cancer risk, while surveillance uses molecular biomarkers as alternative to oesophageal biopsy. It is inferred that chemoprevention holds the answer to BO but must also need epidemiological studies to set change of oesophageal cancer.
